As consideration for participation in a class or activity offered by Zohar Dance Company I hereby agree that I, my spouse, children, assignees, heirs, distributes, guardians and legal representatives will not make a claim against, sue or attach the property of Zohar Dance Company, or any agent employee, or member thereof, for injury or damage to my child(or legal ward) or self, whether resulting from the negligent acts, or howsoever otherwise caused, as a result of our participation in said class or activity. I hereby release Zohar Dance Company and its agents, employees, and members from all actions, claims or demands that I, my spouse, children, assingees, heirs, distributes, guardians and legal representatives now have or may hereafter have for injury or damage resulting from my or my child's or legal ward's participation in said class or activity. Participants and his or her legal guardian consent to the use of his or her likeness n photographs or video taken by Zohar Dance Company for advertising and other promotional purposes. NO REFUNDS unless class is canceled due to low enrollment. All personal absences are forfeited. A minimum of 8 students must be registered. Maximum of 12 students.
